RS assists with transitions from the hospital or the HCC to Independent Living. The department can coordinate services for activities of daily living for the Resident. Assistance may be offered through Companion Services or the support of friends and family. RS will remain in close contact with Health Care to follow up on the Resident’s current condition and to plan the recovery at home.
