Residents are requested to provide the RS office with the most recent copy of a Power of Attorney (POA), Medical Power of Attorney (MPOA), Advance Directive for Physicians, and Out-of-Hospital Do Not Resuscitate (DNR) (if applicable) for use in case of emergency. Forms are available in the RS office and on the Resident Portal.
