Transferring Between Levels of Care

The HCC team works closely with Resident Services to facilitate moves from Independent Living (IL) to AL or HCC when the ARC Resident desires or requires a higher level of care. The HCC interdisciplinary team will also assist in transferring the Resident to HCC if an AL Resident no longer qualifies for AL. Resident Services or the HCC is available to discuss Resident needs and/or supportive services.
