The HCC has a full-time Social Worker to assist Residents with Power of Attorney, Medical Power of Attorney, Advanced Directives, Out-of-Hospital Do Not Resuscitate (DNR) funeral planning, and transfers between levels of care. The Social Worker also organizes care plan meetings for the Residents of the HCC. The Social Worker works in the HCC office.
