Upon entry, Residents are required to record a statement showing the location of wills/trusts and the name and address of Executor/Trustee. Residents provide a copy of the Power of Attorney, Medical Power of Attorney, and Directive to Physicians and Family or Surrogate (Advance Directive) upon moving into the community. If a Resident has a State of Texas Out-of-Hospital Do Not Resuscitate Order (OOH-DNR), the ARC requires a copy as well. These forms are kept on file in the Resident Services’ office.
